Patient engagement with digital disease management and readmission rates: The case of congestive heart failure

Congestive heart failure (CHF) is one of the most common diagnoses in elderly United States Medicare (⩾ age 65) population. This patient population has a particularly high readmission rate, with estimate 6-month rate topping 40%. The rapid rise mobile health (mHealth) presents promising new pathway for reducing hospital readmissions CHF, and, more generally, management chronic conditions. Using randomized research design and multivariate regression model, we evaluated effectiveness hybrid mHealth model—the integration remote monitoring an applied technology digital disease platform—on 45-day patients diagnosed CHF. We find 78% decrease likelihood CHF who were assigned to platform as compared control.
